The php5 port does not list pspell as a variant:

PHP is a widely-used general-purpose scripting language that is especially suited for developing web sites, but can also be used for command-line scripting.

For you to add pspell support yourself you need to resolve any aspell dependancies and manually add --with-pspell=${prefix} to /opt/local/var/macports/sources/rsync.macports.org/release/ports/www/php5/Portfile, uninstall and reinstall php5.

It would be great if the php5 port could offer pspell support as a variant which will also resolve aspell dependancies.
