I commented
#configure.cflags-append "-I${prefix}/include"
because I suppose it is not longer necessary.
Disabled djview (--disable-djview):
- djview hasn't been installed previously anyway.
- I suggest to create separate port for djview (http://sourceforge.net/project/showfiles.php?group_id=32953&package_id=220347), that would link against qt4-mac and would be installed in Applications/Macports/

I've created a slightly different portfile patch - it cleans up the old one a bit more and keeps the qt3-based viewer as a variant.
