Tried upgrading cairo:

$ port installed cairo
The following ports are currently installed:
  cairo @1.8.6_4+macosx+no_x11+quartz (active)
  cairo @1.8.8_0+macosx+no_x11+quartz

Is this from cairo-devel or something?

cairo-devel, which apparently contains the missing file, requires libpixman 0.15, which the version of cairo needed by the versions of pango, gtk2, ghostscript can't use.
