Please try my update to this port (https://trac.macports.org/ticket/47329) with port:qt5-mac-devel (available through here).

qt5 and qt5-creator have undergone significant revisions of late.
Is this problem still occurring with the same error?

It would be very difficult to fix this problem now since qt5 and qt5-creator have undergone significant revisions since its creation.
If this problem persists in the latest versions, please feel free to reopen.
