Documentation should be one HTML page per chapter, not per section

Description
The documentation is currently split into one HTML page per sect1, rather than one HTML page per chapter. This mens that there are many pages which are only a paragraph long, and really logically go together with the pages that follow them. It should probably be one HTML page per chapter. Patch attached.
