(Hello again ryandesign.) I am also experiencing this issue. OS X 10.8.1, Xcode 4.4.1, "fresh" installation of MacPorts. Let me know if I can help.

qt3 is failing to build on my system 10.8.1, not specifically related to libpng 1.5, as this ticket suggests.

The following patch fixes the problem: https://github.com/gnu-andrew/qt3/commit/fcdbfafcb17fcc1958bd25c08894cff08b1e4341
Unfortunately the patch does not apply cleanly to the version of qpngio.cpp in MacPorts, but completely replacing the file with https://github.com/gnu-andrew/qt3/blob/fcdbfafcb17fcc1958bd25c08894cff08b1e4341/src/kernel/qpngio.cpp seems to works.
