Cal brought this up in comment:ticket:38198:6 and I had been thinking of it myself independently before, so I decided to open a separate ticket for it. Anyway, it would be useful to specify dependencies that are only used when running tests, such as expect, for example. (This is kind of similar to my request in #38112 btw, just for a different phase)

I see that pspp-devel has had a commented-out depends_test thing in its portfile for a while now:
# depends_test port:p5.12-text-diff
So there's another case of someone else independently having this idea... (edit: specifically snc... he should get on cc for this ticket, too, considering he also brought it up on IRC)
