Add link to MacPorts ticket creation guide on Trac "New Ticket" page

Since we have  a guide on how to file tickets for MacPorts, it'd be helpful for the new ticket creation page itself to have a link to it. Obviously we'd need to keep an eye on this link so that it doesn't break, but I don't know how this can be done in an easy-to-maintain way.

Assigning to jmpp since there may need to be a little discussion with MacOSforge on how this should look; feel free to re-assign to whomever would be more appropriate.

Thanks for the ticket Maun Suang! I'm switching it to the "server/hosting" component, so that it's added to my  customized search of hosting related requests (cf. HostingRequests).
