qt4-mac: QAudioInput and QAudioOutput are broken

Up to and including version 4.6.3 of Qt, the QAudioInput and QAudioOutput classes of QtMultimedia are barely usable on OS X (Snow Leopard for sure). The issues have been fixed upstream and the fixes should be present in Qt 4.6.5, but it might be worth backporting the fixes. The attached patches do just that.
