Yes, I'm the original submitter. I updated my Trac account to use the same address I put in the port file.
I'll submit diffs in the future as requested.

Great, thanks. Committed in r95574. I also added a library dependency on libtool because it does link with libltdl.dylib.

I've attached a diff of this update. I also switched to using the preferred rmd160/sha256 checksum pair.
Are you the maintainer? The email address you used to report this ticket is different from the listed maintainer email address, but I see a similarity.
